Peta krajiška kozarska (ili kozaračka) brigada je formirana 22. septembra 1942. godine u selu Paležu, na Kozari. Formirana je od boraca Drugog krajiškog NOP odreda "Dr. Mladen Stojanović" i na dan formiranja je imala četiri bataljona sa oko 1.100 boraca.

Brigada je 9. novembra 1942. ušla u sastav Četvrte krajiške divizije, a 1. juna 1943. godine u sastav Jedanaeste krajiške divizije.

Prvi komandant brigade je bio Josip Mažar Šoša, narodni heroj, a politički komesar Boško Šiljegović, narodni heroj.

Prvi bataljon - komandant Žarko Zgonjanin, politički komesar Dragan Marin

Drugi bataljon - komandant Mirko Pekić, politički komesar Pero Čurkuz

Treći bataljon - komandant Petar Mećava, narodni heroj, politički komesar Joco Marjanović

Četvrti bataljon - komandant Rade Kondić, narodni heroj, politički komesar Mihajlo Gačić
