Die Prva Liga RS 2018/19 war die insgesamt 24. Saison der Republika Srpska und die 17. als nur noch zweithöchste Spielklasse Bosnien-Herzegowinas im Fußball der Männer. Sie begann am 11. August 2018 und endete am 8. Juni 2019.

Meister wurde der FK Borac Banja Luka, der damit den sofortigen Wiederaufstieg schaffte.

Modus

Zu den verbliebenen neun Mannschaften aus dem Vorjahr waren aus der Premijer Liga der Absteiger FK Borac Banja Luka und aus der Druga Liga die beiden Aufsteiger FK Alfa Modriča und OFK Sloga Gornje Crnjelovo hinzugekommen.

Wie im Vorjahr wurde die Saison in einem zweistufigen Format durchgeführt. Zunächst spielten die zwölf Teams an 22 Spieltagen aufgeteilt in einer Hin- und einer Rückrunde jeweils zwei Mal gegeneinander. Danach qualifizierten sich die besten sechs Vereine für die Aufstiegsrunde, in der der Aufsteiger ermitteln wurde. Die unteren sechs Mannschaften ermittelten in der Abstiegsrunde drei Absteiger. Die Ergebnisse aus der Vorrunde wurden in die Aufstiegs- bzw. Abstiegsrunde übernommen.
