血小板第4因子（けっしょうばんだい4いんし、英: platelet factor 4、略称: PF4）は、CXCケモカインファミリーに属するサイトカインで、chemokine (C-X-C motif) ligand 4（CXCL4）という名称でも知られる。血小板の凝集中、このケモカインは活性化された血小板のα顆粒（英語版）から放出され、ヘパリン様分子の効果を緩和することによって血液凝固を促進する。こうした機能のため、創傷治癒や炎症に役割を有すると予測されている^[5]。通常はプロテオグリカンとの複合体として存在している。

ヒトの血小板第4因子をコードするPF4遺伝子は、4番染色体（英語版）に位置している^[6]。

機能

血小板第4因子は70アミノ酸からなるタンパク質で、活性化された血小板のα顆粒から放出されてヘパリンと高い親和性で結合する。その主要な役割は血管内皮表面のヘパリン様分子を中和することのようであり、アンチトロンビン活性を局所的に阻害し血液凝固を促進する。血小板第4因子は好中球、線維芽細胞や単球に対する走化性因子であり、CXCR3Bと呼ばれるCXCR3のスプライスバリアントと相互作用し、おそらく炎症や創傷治癒に役割を果たしている^[5][7][8]。

臨床的意義

抗凝固薬ヘパリン投与に対する特異的自己免疫反応であるヘパリン起因性血小板減少症（HIT）において、ヘパリン:PF4複合体は抗原として機能する^[9]。PF4に対する自己抗体は、ヘパリンの投与がなくともHITに似た特徴を示す血栓症の患者でもみられる^[10]。

PF4は、間質性肺炎を有する全身性強皮症（英語版）の患者で増加している^[11]。

ヒトの血小板第4因子は、赤血球内のマラリア原虫の食胞（英語版）を選択的に溶解することで殺作用を発揮する^[12]。
