It's pretty simple. For the first ref, make the ref just like you normally would, only instead of using <ref>...</ref>, use <ref name="NAME">...</ref>, where "NAME" is any name you want to use for the ref (the only rule is the name can't be a number...it can be letters and numbers but not just a number). For every other time you want to use that ref, just type <ref name="NAME"/>, with nothing inside it, and using the same name as before. Just upload it as normal, and in the "Summary" part of the upload page, paste in the Non-free use rationale template. The full text of the template (and what you will be pasting in) is as follows:

Just paste that in and fill in all of the fields. For an example of what it looks like when it's done, here is a fair use image I uploaded a while ago using that template.

I fixed that back on Nov 15, and it's still showing up fine now. If it's not showing up for you, try bypassing your cache (if you're using Firefox, you can do that by clicking the Refresh button while holding down the Shift key}.

I don't know else you can do, but it's definitely something local to your computer, because the image is showing up fine here. Ok, I took it back out. Good catch. To be honest, I didn't even read the magazine article on that page, because the image was taking forever to load so I just assumed "From the Ashes" would be mentioned. After your message, I did a Google search and there's not really any mention of it, so it seems to be all speculation.
